The
Woodmere Rehabilitation and Health Care Center emphasizes an interdisciplinary
approach towards attaining quality outcomes for a variety of diagnoses
including, but not limited to, orthopedic injuries, CVA, SCI, Cardiopulmonary
conditions, and Neurological disorders. Rehabilitation disciplines
including Physical and Occupational Therapies, Speech-Language Pathology,
Therapeutic Recreation, Social Services, and Nursing work together
to achieve maximum independence for the residents they serve.
Goals are established for each resident based on a collective agreement
made by the interdisciplinary team, the resident and family members.
Complete discharge planning is provided with emphasis on adaptive
equipment, home therapy, and home health services as needed to ensure
a safe discharge.

Physical Therapy
Treatment plans are established for each resident based on their
medical diagnosis and history, prior level of function, and most
importantly their personal goals. Treatment techniques include therapeutic
exercises, balance exercises, and neuro-facilitation techniques.
Emphasis is placed on pain reduction, increasing range of motion
and muscle strength, improving transfers, ambulation, balance, and
reducing the incidence of falls.

Occupational Therapy
The Occupational Therapist is committed to increasing the resident's
independence in activities of daily living (ADLs) such as bed mobility,
dressing, feeding, toileting, and grooming/hygiene, Prerequisites
for this includes increasing upper extremity range of motion, muscle
strength, normalizing muscle tone when necessary, and providing
adaptive equipment or assistive feeding devices.
Residents who are wheel-chair bound are assessed for the appropriate
type of wheelchair and proper positioning to prevent abnormal posture
which could lead to skin breakdown and contractures. We teach safety
awareness techniques for independence in wheelchair mobility. To
ensure independence in ADLs, we strive to strengthen and/or increase
cognitive skills.
A major function of Speech and Language Therapy is to facilitate
communication. Human interaction and communication are particularly
important for maintaining the quality of life for older individuals.
Caregivers and staff work as a team and view each resident as a
whole person taking physical, psychological and communication abilities
into consideration. Caregivers and staff are always reminded to
allow time for responses and look for nonverbal expressions as well
as verbal ones. We make use of as many alternative communication
techniques as possible.
After a complete speech, language, and dysphagia evaluation is
administered an individual treatment plan is developed and implemented
to fill the needs of each resident. Our ultimate goal is to restore
each resident to their optimum potential and improve the quality
of their lives.
