The CSH Long Term Home Health Care Program (LTHHCP)
serves the acute and chronically ill, the frail
elderly and individuals with comprehensive care
needs. A broadly-based team of health care professionals
provides assessments, visits and delivers ongoing
care in the home.

The Lavinia Rasweiler Adult Day Health Care Program promotes maximum functioning and health capabilities of clients while contributing to their self-esteem. Additionally, respite and support for families and caregivers is realized. The program offers comprehensive coordination of nursing, social work, therapeutic recreation, rehabilitation therapy & dietary services delivered by a highly skilled, professional day health care team, seven days a week.
