Cortlandt Healthcare is a lovely 120-bed facility situated on park-like grounds. Patients, residents and visitors alike appreciate the beautifully decorated, homelike and friendly atmosphere and take comfort in knowing the latest in technological advances are there if needed.

Over the last number of years, we have expanded and redirected our core services to include sub-acute care and rehabilitation. Sub-acute care is a relatively new term in the field of health care, its origins stemming from traditional hospital care that is know as acute care. Most commonly provided after your hospital stay, sub-acute care is delivered on an inpatient basis and is designed for individuals who require additional care prior to returning home. This may include:

• I.V. Therapy

or a combination of these. Sub-acute care is generally more intensive than that found in a traditional nursing facility, yet less intensive than found in a hospital setting. This makes it an effective and affordable health care alternative!

Each patient at Cortlandt Healthcare receives an individualized care plan formulated and continually monitored by our dedicated and highly skilled Interdisciplinary Team. At Cortlandt Healthcare, that team consists of:

Together with the physicians, this team is fully equipped to handle all levels of care including medically complex cases.

We emphasize the importance of family and friends in the patient care process and strive to make them an integral part of the therapy program. A treatment plan is developed, with input provided by family and friends, to ensure all areas of care are addressed. Family education and training is also provided to assist with the patient's transition back to independent living.

We offer a full calendar of therapeutic recreational activities, including special trips and outings. Religious services, visiting groups and pet therapy is regularly offered as part of the schedule.

Visitors (including children) are welcome daily from 11:00 am - 9:00 pm. "Symphony", our resident cockatiel, will give you a cheerful greeting when you arrive!

Cortlandt Healthcare also recognizes the importance of providing non-medical, hospitality services along with its quality medical care services. Cortlandt Healthcare's courtesy services program helps to set us apart as one of the best sub-acute providers in the northeast.

Free Courtesy Shuttle Service:
We are proud to offer our new beautiful complimentary Courtesy Shuttle service to assist with patient and resident transportation needs safely and conveniently. This new service provides transportation at no charge to patients for travel from the hospital, to and from dialysis or medical appointments, and for therapeutic recreational outings.

Concierge Service:
Our friendly concierge greets patients upon arrival and serves as a resource during their stay. The concierge assists with a variety of needs, from providing basic personal toiletry items in a cheerful welcome bag to managing the patient email service. The concierge also makes arrangements for the delivery of welcome flowers to each new sub-acute arrival. A popular favorite is the complimentary refreshment cart, offered regularly by the concierge service, which features a variety of drinks, snacks and fresh baked goods. Books and games are also available on the courtesy cart.

Accommodations, Meals and More:
Patient accommodations include both private and semi-private rooms, all furnished with the comforts of home. Individual televisions and telephones are in each room and are provided at no charge for the convenience of our sub-acute patients.

Patients may have their meals in our main dining room or choose one of our three more intimate dining areas. Patients may also choose to enjoy their meal in the privacy and comfort of their room. Arrangements can also be made for family and friends to share a meal with the patient or to celebrate a special occasion.

Our "Beautiful You" beauty and barber services are available for patients and residents by appointment weekly while newspapers and our patient email service are available to help with current events and keeping in touch with loved ones.
