Our interdisciplinary team works together to make sure you
get the kind of care that supports the "whole" person. This
team includes nurses, physiatrists, social workers, and a
variety of therapists including speech, recreational, respiratory,
occupational and physical therapists. After a thorough exam
by your physician, we develop a care plan based upon your
specific needs. We define your therapeutic goals which are
modified as we carefully monitor your progress. So that you're
always informed, you, your family and your doctor are invited
to participate in the planning sessions and are continually
updated along the way. Our goal throughout your stay here
is to help you gain as much independence as possible. If you
will be returning home, our discharge planners will work side
by side with you, your family and physician to make that transition
as smooth as we can.

Subacute
Services Those recovering from an acute
illness, injury, or surgery may no longer need the high-technology
of hospital care, but may not be well enough to go home. Our
goal-oriented subacute program assists those who require special
medical treatments and those who need rehabilitation services.
Our subacute
medical care services support patients requiring postsurgical
recovery, complex wound care, infusion therapy, ostomy care,
isolation care, and pain management.

rehabilitative services offer unparalleled care for those
recovering from stroke, joint replacement, and other surgical

The
Springwood For those who require moderate
assistance and can maintain an independent lifestyle, The
Springwood is an elegant choice. In this upscale wing of The
Claremont, residents accustomed to an independent or assisted
living setting are pampered with extra amenities such as fine
furnishings, private areas and special activities. Tablecloths
grace the dining tables. Our ice cream parlor, salon, library,
aviary, aquarium and Windsor Dining Room are all central to
The Springwood wing.
Respite
Care Family members who care for a loved
one at home often need time to restore themselves for a weekend,
a week, or longer. A respite visit may also help a loved one
in considering a long-term stay. The Claremont provides respite
care for occasions such as these.
Alzheimer's
Care
For those with Alzheimer's disease or other dementias, a safe,
secure, caring environment is necessary. The Greendale Unit
for Alzheimer's care is an intimate setting where soft colors
and gentle patterns calm and soothe. Our staff, specially
trained in Alzheimer's/dementia care, arranges a variety of
daily activities to engage residents and appeal to every personality.
Our social service staff offers support and counseling to
families.
Special
Neurological Services Our specially trained
staff offers interdisciplinary services for those needing
care for Parkinson's disease and stroke recovery. Our social
services team offers support, counseling and educational information
to families and the community.
Hospice
Care When terminally ill patients can no
longer stay at home, we can provide them with the comfort
and compassion of a home-like setting. Our staff is devoted
to supporting all aspects of the individual the emotional,
physical, mental, and spiritual helping them live full
lives one day at a time. Consulting hospice nurses, social
workers and clergy provide additional support for residents
and their families.
In-House
Dialysis Services Inpatient dialysis offers
you schedule flexibility in the care focused, comfortable
setting of The Claremont. Physicians and therapists can integrate
dialysis into your overall care, whether you're at The Claremont
for long-term care or short-term rehabilitation. Our integrated
program also eliminates your commuting costs, and travel time
can instead be used for other care needs. Inpatient dialysis
can substantially reduce your short-term rehabilitation stay.
Outpatient
Care
Our outpatient physical therapy and occupational therapy are
open to the public. Pain management and lymphedema services
are available.
