| Description |
A NURSING HOME ALTERNATIVE
Community LIFE is a community-based alternative to nursing home placement, which enables the frail elderly to remain in their own place of residence, despite medical and functional frailty.
The 3 mainstays of the program are:
Primary Medical Care
In-Home Services
Adult Day Care
Other services include medications, transportation, rehab therapies, recreational therapy
Community LIFE is an integrated health plan, which arranges a full array of health and social services 24 hours a day, 7 days a week, 365 days a year.
The enrollment process involves an options assessment by the Allegheny County Area Agency on Aging to determine clinical eligibility, and approval through the County Assistance Office for Medicaid eligibillty. This process may take several weeks to enroll an individual. |

Participants are Medicaid and Medicare eligible, or have the ability to private pay if ineligible for Medicaid.
In order to enroll into Community LIFE, the individual must be preapproved for Medicaid(not MA pending).
The Intake Specialist will assist the applicant with application for Medicaid. |
| Policies |
· Must be 60 years of age or older
· Must live in Service Area (see below)
· Must be eligible for Medicaid or have ability to private pay
· Must be deemed nursing facility eligible by the Allegheny County Area Agency on Aging
· Must be able to live safely in the community with the services provided
