Subacute Rehabilitation Services
Subacute rehabilitation services are primarily for patients who do not need or cannot tolerate more intense and comprehensive inpatient therapies and treatments.
Subacute care generally serves as a bridge for two types of patients. It fills the gap between the acute care hospital and the rehabilitation facility for patients who need medical rehabilitation but are not ready to begin a full, comprehensive rehabilitation program. However, these patients do not require acute care hospital services.
Subacute care also helps patients who have progressed beyond needing a full, comprehensive rehabilitation program. However, these patients still require some additional rehabilitation support and services that cannot be provided solely on an outpatient basis. All of the many services provided to inpatients and outpatients are also provided to subacute rehabilitation patients but at varying degrees depending on each person's needs.
Comprehensive services include:
- Nutrition prescription and management
- Geriatric and restorative nursing
- Intravenous therapies
- Hyperalimentation
- Tracheostomy care
- Therapeutic recreation
- Case management
- Medical management of decubitus ulcer wound care
- Physical restoration (low intensity) for patients with cancer, orthopaedic injuries, cardiovascular and neurological problems or chronic obstructive pulmonary disease
- Catheter care and intermittent catherization
- Patient/ family teaching
