Our Social Services Department has five Certified Social
Workers, one of whom holds a Degree in Psychology. The social
work staff plays a vital role for our residents in their
transition from independent living to life at Somers Manor
Nursing Home. Social Work intervention begins from the admission
process and continues through the resident’s stay
with us. If the resident’s goal is to return home,
the Social Workers will work assist the resident, family
members and community services personnel to facilitate a
safe discharge back to the community through interdisciplinary
planning.
While at Somers Manor, residents are encouraged to participate
in the group services provided by the Social Services Department.
These services are in addition to the individual assistance
and support that are regularly provided to residents and
families. Group support services are also provided for family
members. These groups afford the opportunity for family
members to share common concerns and experiences with the
professional staff. And remember, whether you are speaking
with a Social Worker or participating in any of our support
groups, you can be assured that whatever you say will be
kept strictly confidential. Your right to privacy is respected
by all staff and volunteers at our facility. This allows
for an open dialog which is vital to meeting the resident’s
needs while living at Somers Manor.
Our Social Work Department hours are Monday to Friday from
8:00am to 7:00pm and also Saturday from 10:00am to 2:00pm.
