The application process for inpatients begins with the completion and submission of the applicant's admissions form and various other documents as required by Federal and New York State laws. These documents are known as the Patient Review Instrument (PRI) and the pre-admission medical SCREEN for appropriate placement. These forms fully describe the type of care and services required by the applicant. For prospective residents who are applying from a hospital, the hospital staff will complete these forms. For applicants living at home, the forms are to be completed by an authorized nursing agency such as the Visiting Nurse Service (VNS). Laura Medeiros, the Somers Manor Case Manager, will provide information and assist you with your application. And if you can’t travel, she will come to the hospital or the community in which you reside.

Upon receipt of your application, the Somers Manor Admissions Committee reviews the completed PRI and SCREEN forms for approval. The committee's primary focus is to determine whether we can properly meet the applicant's health care needs. On occasion, we may request additional medical background and information on healthcare needs to help us with this review process.

The next step is a review of the applicant's financial status. Somers Manor accepts private payment, Medicaid, Medicare and third party insurance. Our staff is available to guide and assist prospective residents in these matters. They can also help you with your spend down process and in applying for Medicaid.

Prospective residents, designated representatives, family members and sponsors are all cordially invited to call for information and assistance.
