WHAT IS SUB-ACUTE CARE?
"'Sub-acute care" is a relatively new term in the field of health
care. Its origin stems from traditional hospital care which is known
as acute care. Most commonly provided immediately after your hospital
stay, sub-acute care is delivered on an in-patient basis and is
designed for individuals who require additional care prior to returning
home.
Sub-acute
care is generally more intensive than that found in a traditional
nursing facility yet less intensive than found in an acute setting.
This makes it an effective and affordable health care alternative.
Sub-acute care is an extremely vital component of a patient's total continuum of care. Each patient receives an Individualized Care Plan which is carefully coordinated and monitored by an interdisciplinary team. By incorporating sub-acute care into an individual's overall care program, a patient has a much greater opportunity for maximal recovery.
