SOUTH SHORE
HEALTHCARE ... FREEPORT, NEW YORK
We're
Between the Hospital and Home!
We know that
making a choice about a health care provider is an extremely important
one. When it comes to deciding about sub-acute care however, the
choice is clear. South Shore Healthcare in Freeport leads the way
as Long Island's premier facility for sub-acute services and has
served as a key health care provider in the community for over 45
years.
Our beautiful
100-bed facility was completely renovated and redecorated in 1998
to reflect the warmth and caring of the staff. The facility is bright
and clean with a friendly, home-like atmosphere. We are fully equipped
with the latest in state-of-the-art technologies and are proud to
have one of the largest therapeutic rehabilitation areas on Long
Island. At South Shore Healthcare, patients can work towards their
goals along with top rehabilitation professionals in our spacious,
sun-filled therapy room.

One of the key
programs at South Shore is our respiratory and ventilator-weaning
program. Patients are monitored by Board Certified Pulmonologists
and specially trained Respiratory Therapists. We are highly regarded
within the medical community for this program and its outstanding
proven success rate with our patients. The facility is fully equipped
with in-wall oxygen giving patients who require oxygen access to
most areas of the building and grounds.
Patient accommodations include both private and semi-private rooms, all tastefully decorated and furnished with the comforts of home. Individual televisions and telephones are also available for the convenience and enjoyment of our patients.
When it comes
to dining, patients have their choice of locations. Patients may
dine in one of our two spacious dining rooms, relax with a meal
in the privacy and comfort of their room, or weather permitting,
enjoy a meal out on our lovely shaded patio. Special arrangements
can also be made for family and friends to share a meal with the
patient.

Under the direction of a physician, patients may receive regular meals or a therapeutic diet all carefully prepared by one of our in-house chefs with the guidance of a registered dietician. Kosher and Kosher-style meals are also available upon request.
There is a full calendar of recreational activities to enjoy, or patients can pursue their own activities and interests. Trips and outings are often offered on the activities schedule.
Patients at South Shore Healthcare are cared for by a broad complement of fully credentialed physicians. Each patient receives a carefully individualized Care Plan formulated and continually monitored by our dedicated, skilled Interdisciplinary Team. At South Shore, that team consists of:
- RN Case Managers
- Registered and Licensed Nurses
- Pulmonologists
- Respiratory Therapists
- Oncologists
- Physiatrists
- Psychiatrists/Psychologists
- Physical Therapists
- Occupational Therapists
- Speech and Language Pathologists
- Recreational Therapists
- Certified Nursing Assistants
- Social Workers
- Nutritionists
- Audiologists
Together with the physicians, this team is fully equipped to handle all levels of care including medically complex cases.
We emphasize the importance of family and friends in the patients' care plan and strive to make them an integral part of the therapy program. A treatment plan is developed, with input provided by family and friends, to ensure all areas of care are addressed.
We are extremely
well regarded in the medical field and maintain referral relationships
with most Long Island hospitals, many physician groups, and health
facilities nationwide.
