SOCIAL WORK
Our staff of Social Workers is an integral link between resident,
family and other members of the health care team and the community.
We offer on-going support and comprehensive care management, including
discharge planning. All patients/residents are assigned a social
worker, who acts as a resource person, advocate and counselor. Social
Workers assist the resident and family in adjusting to their admission
and/or illness. A psychologist is also available for in depth and
ongoing counseling. Our social work staff will assist residents
and families in making a smooth transition from home to the facility,
or back to the community. Residents and their family members are
encouraged to participate in the life of the facility, including
regularly scheduled care planning meetings. There are also active
Resident and Family Councils.
HOSPICE
Buena Vida provides Hospice service in association with the Metropolitan
Hospice of Greater New York to qualifying residents who have been
diagnosed with a terminal illness. Hospice services are geared to
comfort rather than curative care. The Admissions/Social Work Department
can answer your questions related to Hospice care.
Care for the terminally ill residents is provided in collaboration
with
Metropolitan Hospice of Greater New York, a nationally recognized
Hospice
Association. This program is sensitive the special needs of those
at the
end of life and their family including palliative care, and bereavement
counseling.
