We are aware that placing someone in a nursing facility can be complex, whether you are seeking short-term rehabilitation or continuing care. Our highly trained admission staff are available to guide you in admitting your loved one.

If applying from a hospital, contact your Social Worker/Discharge Planner, who will arrange for a Patient Review Instrument (PRI) and Screen. The PRI and Screen forms are New York State required admission forms and provide most of the medical information we need to process an application for admission. You should be aware that hospital transfers often take place rapidly, so if you want a tour it should be arranged as soon as possible.

If you are applying from home please contact our skilled admissions staff who will guide you through our admissions process. If you are requesting a transfer from another nursing facility contact the social worker, who will arrange for a PRI and Screen and facilitate the transfer with one of our admission staff.

We always recommend a tour, which can be arranged during business hours by calling the Admissions Department. Weekend tours are also available through our security department.

Payment: Buena Vida participates in the Medicare and Medicaid programs and we accept some managed care. All payment and insurance concerns can be addressed by our staff.
