and rehab-certified nursing assistants, work to assist our
residents in achieving their highest level of independence.
Our goal is to discharge them back to their home whenever

Fact Sheet
At
Elm Brook HealthCare & Rehabilitation Centre our comprehensive
in house dialysis program offers convenience and comfort to
our residents. The dialysis program provides identical services
to those given at either a hospital or out-patient dialysis
center. Fully trained nurses and technicians monitor and perform
the services under the direction of your current or our in-house
nephrologist.
