Oakton Pavilion has been on the cutting edge of all the latest developments in long therm care since 1980. WOUND CARE PROGRAM
One of the most devastating occurrences in the infirmed elderly is the breakdown and compromise of skin integrity. At Oakton Pavilion, we take pride in our intensive program to prevent pressure sores. Upon admission the nurse assesses each patient with input from the entire interdisciplinary team regarding his or her risk for skin breakdown. Following this assessment, an individualized plan is implemented.
Components of the Wound Care/Skin Integrity Program include:
- Specialty dressings
- 24 hour attention and treatment
- Specialized airflow mattresses or others, if indicated
- Individualized dietary intake procedures by our own Registered Dietician
- Highly trained nurse specialists on all shifts
- Individualized tracking and documentation of outcomes
IV THERAPY
Skilled long-term care facilities are being utilized for many patients in order from them to complete a course of complex intravenous therapy, often in highly resistive cases. In many cases, these patients also require additional skilled nursing nursing services such as isolation procedures, and are at risk for other medical problems.
At Oakton Pavilion, guidelines from the Center for Disease Control and the Occupational Safety and Health Administration are strictly adhered to. On-going Infection Control monitoring creates an environment in which complex IV therapies can be administered.
As part of our IV Therapy Program, we offer:
- Complete range of IV antibiotic therapy
- 24 hour and STAT delivery from our pharmacy
- PICC lines and other modes of IV infusion
- RN consultant available for individual cases
- Routine and STAT laboratory testing
REHABILITATIVE / RESTORATIVE THERAPY PROGRAMS
At Oakton Pavilion, rehabilitative and restorative programs are a part of everything we do. Returning patients to maximum self reliance requires the involvement of each of the professional disciplines. Beginning with our professional therapies - Physical, Occupational and Speech - the patient starts a continuing process which will restore self-esteem and the ability to function at the highest practical level.
Highlights of this program are as follows:
- Formal, professional, comprehensive assessment of all functional abilities and activities of daily living
- Design of individualized professional therapies utilizing the Physical, Occupational and Speech programs.
- Professional therapy programs as intensive and as frequent as the patient's needs dictate
- Strength training
- Ambulation training
- Range of Motion exercises
- Hydro and heat treatments
- Transfer training exercises
- Physical and Occupational Restorative programs by our own Rehabilitation nurses, Rehabilitation aides and direct care staff
- Family involvement in understanding and assisting in the progress of the patient
- Recommendations for home care follow-up
