Us:
Lakeview Nursing and Rehabilitation Centre is a privately-owned
skilled nursing facility, unique in our patient-centric
approach to healthcare. We customize our services to
meet each of our resident’s personal preferences,
special needs, strengths, and capabilities.
Our multi-disciplinary approach to each resident’s
plan of care is based on a foundation of superior communication,
proactive thinking and the desire to excel. Each Lakeview
team member has been thoroughly trained to have the
knowledge, attitude and skills necessary to promote
the wellness of our residents through mind, body and
soul.
Lakeview is a cornerstone of the Lincoln Park/ Lakeview
area. Our location is close to Chicago’s best
hospitals and we are easily accessible by car, train
and bus. We also offer free valet parking to our visitors
and medical team.
Admissions to Lakeview are stress-free and handled
with the highest level of sensitivity and respect. We
handle all of the paperwork so our residents can focus
on their wellness.
Our Services:
• Short-term Therapy: We
are committed to rehabilitating our residents and
restoring them to their maximum level of physical
and mental functioning in the most independent setting
possible. Our therapy services include:
- Physical Therapy: We work to reduce or prevent impairments,
compensate for physical limitations and accelerate
recovery time from illness or injury.
- Occupational Therapy: We teach the physical and
cognitive skills each resident needs in order to resume
as independent a lifestyle as possible. Daily activity
skills include eating, dressing, and management.
- Speech Therapy: Lead by a certified speech language
pathologist, our speech department concentrates on
speaking, hearing, swallowing and language comprehension.
- Post-operative Orthopedic Therapy: Our therapists
utilize advanced training in manual techniques and
exercise to treat patients with musculoskeletal dysfunctions.
Patients are evaluated and treated to determine the
source of their pain.
- Post-cardiac Therapy: We help ensure a successful
recovery for residents who have had stable angina,
heart attacks, bypass surgery, angioplasty, valvular
problems and congestive heart failure. We assess how
much is safe for each resident and how quickly they
should progress.
- Independent and Safe Transfer programs: This better
prepares our residents for what lies ahead and for
their period of convalescence. This lessens the chances
of pain, tiredness, loss of mobility, and grief over
loss of previous abilities.
- Wii Therapy: This is a fun way to improve our resident’s
range of motion.
• Wound Care: Our nursing team
is specially trained in the field of wound care and
adhere strictly to the medical guidelines for skin
care treatment. We are dedicated to maintaining healthy
skin by keeping our residents clean and dry, performing
daily checks for pressure ulcers and properly adjusting
the position of bedridden residents and those restricted
to wheelchairs. Our staff members also follow the
Center for Disease Control’s guidelines to prevent,
control and eliminate infections.
• Alzheimer’s and Dementia Care:
24 hour care is provided within a special unit of
our facility, designed to sensitively care for all
stage of dementia and address the special needs of
those with Alzheimer’s or other memory-related
disorders. This unit is a secure environment to prevent
wandering from the facility equipped with entry/exit
keypads.
• Palliative Care: The goal
of palliative care at Lakeview is to relieve the pain,
symptoms and stress of serious illness – whatever
the prognosis. It is appropriate at any point in an
illness and can be delivered along with the treatments
that are meant to cure. We provide better understanding
to our residents of their condition, choices for care
and improve their ability to tolerate medical treatments
and enjoy everyday life.
• Respite Care: Even though
many families take great joy in providing care to
their loved ones so that they can remain at home,
the physical, emotional and financial consequences
for the family caregiver can be overwhelming without
some support. Respite provides the much needed temporary
break from the often exhausting challenges faced by
the family caregiver. We offer temporary care for
family members who might otherwise require permanent
placement outside the home.
• Care Plan Meetings: All relevant
staff members from our therapists to our nutritionist
attend regular care-planning meetings to discuss every
aspect of an individual resident’s needs. We
strongly encourage the resident and their family to
participate in the care planning meetings.
• Daily Activities: Our on-site
Director of Activities plans special events for every
day of the week. Our residents have the option to
participate in off-site trips, visits from volunteer
groups, holiday parties, movie nights and much more.
We are also ever-expanding the bilingual staff and
incorporating culturally-rich activities to better
serve the varying cultures of our residents.
